This Custom Engraving Form is to be used only after an image has been submitted for evaluation using the Custom Engraving Information link in the top horizontal Engraving Services drop-down menu and a Set-Up conversion quote has been emailed to you with an assigned ID#.

This engraving service is only for a custom image or logo engraving on a pocket watch with plain covers.

Please make sure that the image/logo is appropriate for the watch ordered and that it does not conflict with any other engraving request.
